[Plugin: Jetpack by WordPress.com] Contact Form Not Displaying

  • I am also trying to use the contact form on my site.

    I only have a few plugins running:
    Jetpack
    Awesome Flicker Gallery
    Askimet

    I have just downloaded WP 3.5, but this hasn’t improved the situation at all.

    However, the contact form within jetpack does not appear on my site. I added the code through the button. It reads:
    in HMTL:

    <p style=”text-align: center;”>Please contact us using the form below.</p>
    <p style=”text-align: center;”>[contact-form subject='CONTACT FORM'][contact-field label='Name' type='name' required='1'/][contact-field label='Email' type='email'/][contact-field label='Who is this email for?' type='select' options='Madison,Sylvie,Nick,Jme'/][contact-field label='Comment' type='textarea' required='1'/][/contact-form]</p>
