[Plugin: Gravity Forms Salesforce Add-on] upgraded Gravity plugin now Salesforce plugin stopped

So I’ve upgraded the gravity plugin and now I’m unable to submit a form. I can fill out the form but when I click on the submit button nothing happens: see screencast.
