    It seems like it might be a multi-page issue. Does it work properly when you only have an one-page form?

    Zack now that I’m re-reading your comment, one page not one form. I haven’t tried that let me check.

    OK so here I’ve gone back and removed the page sections, which messed up the form a bit, however still had same issue. My very simple form screencast

    Therefore I’ve gone and created a very simple form: Name, phone, company and description. I’m getting the same issue: two leads being created. And as a matter fact one of the leads is getting both First and Last name placed into the last name. The other lead is has the first and last name correct. But BOTH still do not have the company, phone, or comments coming across.

    I must be missing something or doing something wrong because it really can’t be this hard.

    OK so for posterity here’s the confusion I had. The blog post by webMachanix and the setting for turning on the sfdc integration by the sfdc gravity plugin should not be used together.

    So either use the web-to-lead form method which is out lined in the blog post and be sure NOT to enable the Salesforce integration on the Advance tab of the form setting, screen shot.

    That is the issue with having duplicate leads created. Now onto the two fields which are not being populated.
