[Plugin: Gravity Forms Salesforce Add-on] Settings page?

I’m using the latest version of WP and GF but I’m not finding the setting page for this plugin in the admin. After activating the plugin I don’t see a Salesforce link in the Forms menu in the left column. The checkbox does show up in each form’s advanced settings so it is activated.
