[Plugin: Gravity Forms Salesforce Add-on] Name, Date, and List Field Issues

Thank you very much for Gravity Forms Salesforce Add-on. It is very useful and has saved us a lot of time, though we are still having several issues with it:
1. The Name Field
When we have two GF standard name fields on the same form, only the field closest to the bottom of the form goes into SalesForce. It appears to be overriding the previous name field. Also, the suffix from the name field will not show up in SalesForce. We were able to craft an okay work-around to this problem by just using separate text fields, but they do not look as nice as the standard name field.
2. The Date Field
Entries into the standard GF “date” field will not appear in SalesForce (we use it for DOB). Again, we could fix this by using a plain text box, but it would not look or function as well (e.g. it would not standardize the formatting).
3. The List Field
It appeared from your changelog that your most recent update to this plugin would allow the list field inputs to be entered into SalesForce as a single SF field. However, when we enter data into the list field boxes, all that shows up in SalesForce is the text “Array.”
We tried upgrading the plugin to the most current version, because it appeared that it fixed these problems, but it did not resolve these issues (it did not appear to change anything from our perspective). We even tried deactivating the plugin and deleting it and then downloading the new version, but it still did not change anything.
