[Plugin: Gravity Forms Salesforce Add-on] Form not submitting to Salesforce

I have the “Enable Salesforce integration” option checked on my form, but nothing is getting sent to my salesforce account. My Organization ID is correct, and I’m not getting any errors, but none of the data is being placed anywhere into my salesforce account.

Any ideas?
