[Plugin: Gravity Forms Salesforce Add-on] duplicate leads

I’m getting duplicate leads being generated. And the weird part is part of the form data is on one lead where as the other is not.

Right now what I can figure is one of the leads has most if not all custom fields plus a couple of the standard fields but missing a phone number and the lead description field. The other has the standard fields of name, phone, and description.
