[Plugin: Gravity Forms Salesforce Add-on] Doesn't show up in Forms menu.

Hi,
I’m using Gravity Forms 1.6.3.3.4 and 1.1.3 and for some reason the link to the settings for the Salesforce plugin doesn’t show up anywhere.
Anyone have a similar problem?
