[Plugin: Gravity Forms Salesforce Add-on] Custom Fields Not working

I’ve installed and successfully used it with ease for the standard fields, but custom fields have been extremely problematic. Have followed instructions and attempted several possible variations.

Oddly, it was working fine for about four test submits then stopped working altogether.

Will use the plugin for the standard fields – works beautifully – but will eagerly wait a solve on the custom fields.

Any help or suggestions would be greatly appreciated!
