[Plugin: Gravity Forms Salesforce Add-on] Beginners Help

I’ve installed the plugin on my site correctly and it says it’s integrated. However when I submit the form I don’t see any info in salesforce.
1. Where do the submissions end up in Salesforce?
2. Do I need to create a web to lead form or similar?Here is a link to the from I’m sending
Any help on the setup process is appreciated, I’m a bit lost.
