[Plugin: Gravity Forms Salesforce Add-on] Advanced form settings not showing Salesforce option

I have submitted this issue on another ticket which is now marked resolved and is not.

Having the same problem as @zack, except that I am upgraded to WP 3.4.1, Gravity Forms Version 1.6.4.5.4 and GF SF Add-on Version 1.1.3.
