[Plugin: Google Maps via Store Locator Plus] Theme breaks on load

Hi, I like your simple default theme very much, but it breaks almost immediately on page load. I only noticed this because I was on a dreadfully slow internet connection ?? .

Any assistance you can provide is much appreciated.
