[Plugin: Google Maps Store Locator for WordPress] Store Loator doesn't show up

I insert [STORE-LOCATOR]in page body area, but when I open that page in front page, it displays

Address –Search By City– Albury, NSW Diamond Beach, NSW Manly, NSW Radius 1 mi 5 mi 10 mi 25 mi 50 mi 100 mi 200 mi 500 mi Lots of Locales by Viadat Enter Your Address or Zip Code Above. if (document.getElementById(“map”)){setTimeout(“sl_load()”,1000);}

Anyone can help to get map on the page in place of this text.
