[Plugin: Frontpage-Slideshow] FPSlideshow will not display anywhere on site

Seems like a great plug-in, but I cannot for the life of me get it to work anywhere on my site. I have tried placing the PHP code in the header.php and index.php files and as shortcode within various pages and posts. No matter which method, the only thing that displays is the actual shortcode [FrontpageSlideshow] instead of the actual slideshow. The preview works and displays correctly (it cuts some of the image off even though it is sized to the slideshow dimensions).

Have tried altering the php to work, have read through the posts, all to no avail. I am sure it is some simple thing, but I cannot figure it out after 2 days.
