[Plugin: FormBuilder] How to add image left of text area

Hi,

Im trying to create a feedback page. I want it to have title of the product then below is a text area with an image to the left. No problem with the title but the image, is this possible?

I dont want to create an html table and just put the form to right and images on left. Coz if I do so, everything in the form like title (or single text field) will move to the right also.
