Display form entries in front end

i have a simple task, i want user to fill form and each user can fill form multiple times, i know i can do it by using any form builder tool

now what i want a page which say same user specification like

how many entries a specific user addes in form (only show entreis the user who logged in)

count entries total

we can pay user $5 per entry so if he 5 records submit her total balac will be $25 (or any such simple calculations)

which plugin suites best and what it called?
