Plugin for Employee Applications / Misc. Forms?

Is there a known plugin where you can either create online forms or use existing PDFS for employees to login and fill out items such as an employment applications – fillable pdf or otherwise – as well as other employment related documents such as job descriptions, tax forms, misc. acknowledgements, etc.?

I have searched and found several job board plugins for posting open positions, but nothing for someone to completely apply online for a job.
