Plugin for easy forms which print result?

  • I would like a plugin that lets me:

    * Embed form elements in a post or page (such as text, radio, drop-down, etc.).

    * For instance, one field is “How many bananas do you want?” and the next is “How much is one banana?”, and then the answer “Total cost of your bananas:”, after user presses “submit”, is printed.

    Anybody know of such a plugin or method?
