[Plugin: Fast Secure Contact Form] 2 strange boxes at the top of the form…

Love this plugin! Very customizable and works great.
However (you knew it was coming),at the top of the form there are 2 boxes that show on ALL browsers, but not the tablet. Just above the “Name:” text.
They may be hidden form items, but they aren’t hidden lol
Any ideas? They are definitely part of the form as even with all text removed from the page, they are still there.
