[Plugin: Events Manager] Event form to allow user events

Hi,
I’ve used the shortcode [event_form] unfortunately it completely crashed my template so I cannot use it. The form appears 3 times on the same page, for no reason.
Although it’s said as bêta, I was wondering if you could use it somewhere with no issues ?
