[Plugin: Email Before Download] Endless Spinning after update to WP 3.4

Plugin was working fine until we updated the site to WP 3.4

Now, upon user submission of name and email address, the form just hangs while the spinner just turns and turns. No email is sent with the download and the links are not returned, but we do receive the user’s email.
