[Plugin: Displet Pop] How do you connect this to a form exactly?

So I am a total newb at this WordPress stuff, but I downloaded your plugin and I am just wondering how you attached the actual form code to this plugin for the pop up?

I have several contact forms and some custom code for auto responders, but for the life of me I don’t get how you associate your plugin with those forms. I think you at least need this explanation/directions listed on the installation, FAQ or description.

Your site and examples have a great looking pop up but no info on how you attached the form, html or code for getting the newsletter/custom code.
