[Plugin: Custome Contact Form] Create Style Button page not found

I have been using the default style for my contact form but I now wish to change the width of the form. I have gone to the Style tab, I have entered a unique Slug for the style and I click the Create Style button. It takes me to my website and I get a unable to find page error. The style is not added to the style listing so I have no option to change the forms style. Please help.
