[Plugin: Custom Contact Forms] Suddenly Stopped Sending E-mails

I am running WordPress v3.2.1 since December 2011 and originally installed Custom Contact Forms v4.7.0.4, which worked flawlessly until last week. All of a sudden Custom Contact Forms just stopped sending e-mails submitted from any form. I did not make any changes or install any plugins — CCF just stopped sending e-mails.
After reading many suggestions in these forums, today I successfully installed the WP-Mail-SMTP plug-in. I receive the test e-mails from that plug-in using the settings below, but still do not receive any e-mail from the CCF forms. Then I upgraded Custom Contact Forms to v5.0.0.1 after trying the PHP mail and SMTP settings suggested in these forums, to no avail. The setup is as follows.
The site is hosted on Godaddy shared linux server
