[Plugin: Custom Contact Forms] How do I add an "I agree" checkbox with a paragraph of text

How do I add an “I agree” checkbox with a paragraph of text before the “I agree” – I’m not sure how to add the box of Terms and Conditions.
There must be a way, I’m just not figuring it out.
Obviously I can add the “I agree” checkbox, just having trouble inserting the Terms and Conditions.
