[Plugin: Custom Contact Forms] Custom HTML Forms

I hate to even post this because I should be able to figure it out but I’m lost as to how to create the Custom HTML Form described in the Advanced tab. I’ve tried adding the sample code into a new post in HTML view but nothing shows up except the form title. Also, unfortunately the tooltips for items in the “Advanced” block, such as “Form Action”, “Form Pages” and “Form Access”, display beyond the left side of my browser where I cannot read them.

I have been successful in creating custom fields but that’s as far as I’ve been able to go. The Plugin Usage Manual is supposed to contain detailed instructions but they’re apparently not detailed enough for me. I would appreciate any guidance anyone could give me on this as I’m trying to complete a form that requires the ability to create a custom form.

I don’t have a link to the site because I haven’t gotten the form to work enough to post yet.
