[Plugin: Custom Comment] DOESN'T DISPLAY

After custom fields are added in the admin panel, YES they show up on the page.. But it breaks completely after that!
You can fill out the custom fields..Hit Submit though and it breaks

The Custom field LABELS AND CONTENTS disappear..
