[Plugin: Contact Form 7] Upload not working and odd .htaccess change

When trying to upload a file with CF7, the form goes through fine, and the mail comes in with the mane of the file – but no attachment.
The uploads folder, which is properly permissioned, keeps populating with a new .htaccess file every time we submit a form – the file says “deny from all”.
Any thoughts?
