[Plugin: Contact Form 7 to Database Extension] Shortcodes don't display any data?

Help, please!

I’ve created a form and used it to save data to the database. I can see the data via the dashboard. I created a page and added the shortcode:
[cfdb-table form=”name”]

Nothing displays on that page. No shortcode, no table, no data. Nothing.

Used “title” of form as name. That didn’t return anything, so I tried using the form “id” with the same results.

Set the following settings:
Can See Submission data: Anyone
Can See Submission when using shortcodes: Anyone

Still nothing.

Tested using multiple browsers with the same result.

I’ve obviously missed something here, likely something quite simple, but I don’t know what!
