[Plugin: Contact Form 7] Form isn’t working.

First off you can view the form I’m building here. Now I have filled this out and sent it to myself, but this is all I received im my email…

I also can’t figure out how to change the names of these fields so that when they arrive in my email I can see them. Right now all I’m getting is “Message body” and the subject line of the email is just “your-subject”.

Any help with this would be appreciated.
