[plugin: contact form 7] form change with selection

I’ve tried searching and finally just decided to ask the community.

I’m trying to create a form that changes when a certain selection is made.
ex: phone repair, laptop repair, gaming system repair
when you select phone repair a form comes up with the custom settings to send in your phone for repair.

im not sure how to correctly word what I’m trying to do this would be my first form I’ve tried to create like this. Any help would be appreciated.
