[Plugin: Contact Form 7] ERROR – Contact 7 form shows default fields on actual page

This is surprising! I’ve used this contact form before with great success. However, I just formatted the form on a new site and it shows up on the site with some random default settings, Name, Email and Website.

I created a 2nd form called home form and added that shortcode to the post and it still shows the default form.

Here is the page. I have filled out a very robust form that should be displayed instead. Does anyone know what could be causing this?
