[Plugin: Cimy User Extra Fields] Radio Button on other Page

Hello,
i would like to add some Radio Button Field to a site, that i have to code by myself cause it is created by another plugin.
Therefore i have to add the radio button choices by hand.
How do i manage this?

I found some code for a dropdown but didn′t now how to change it, that it fits with my wishes.

I want to have the radio buttons, that are shown on the profile page to be shown on a other page. Therefore i have to get the code.
