[Plugin: Child Pages Shortcode] Further instructions?

Greetings — I like this plugin a lot so far, but a little bit of instruction on its use would go a really long way toward making it more usable.

Specifically, I need to know:

Where is the “filter hook for template” located, and/or where should this code be placed in my page template?

Where is the actual template file?

Where is the “filter hook for stylesheet” located, and/or where should this code be placed in my page template?

If the “Default source of template” is not located in a separate file, where is it located?

How to create a brand new template, and/or edit the existing template?

Any help would be appreciated. Thanks!
