Plugin busted media uploader

After activating the plugin, everytime I tried to upload an image using the media uploader, I would received “An error occurred in the upload. Please try again later.”. I tried lots of fixed, before I found the cause of the problem. After deactivating the plugin, upload works fine again.

What could be the problem? I would really like to continue using the plugin.
