[Plugin: Bug Library] Captcha not displaying image

I have just installed this plugin to my 3.3.2 wordpress. When I try to create a bug the captcha does not show up. What have I done wrong ?
All in all the getting started guide is really poor on this plugin. Please improve your documentation. For example, the setup of this plugin is in Bugs Library -> Bug Library. Just not intuitive until documented somewhere. Ideally it should be in Settings->Bug Library.
