N.B. Since that’s a paid plugin, the solution really needs to come from their side. (i.e. I’m not going to buy a copy to investigate what they’re doing!).

You don’t have to. The plugin is complex, messed up my site.. I had to waste 1 hour to revert back to normal. I deleted the plugin. You can close this ticket. Btw, this 2fa plugin is the best free 2fa plugin for wordpress. Thank you so much for building it.
