The plugin comes with very minimal styling. In fact, it inherits the styling from the site. As you see from the image you provided the email field is different from the other fields as your theme developers have not defined html5 form fields such as the mail form. So, to change the styling you just need to modify your sites main styles.css file. If you need assistance with any of these do not hesitate to contact us through https://bigemployee.com.

I have looked into the plugin and the date/time format works in the appointment email. However, the values are taken form the Big Contact -> Settings page. There is more information about how to use the correct formatting there. The default values for the Date and Time are DD-MM dd, yy and hh:mm tt respectively. However, if you still experience the same problem please send me a link to your site and your WP/PHP versions.
