Placement after comment textarea and more semantic HTML?

Seems that “Display attachment field” doesnt do anything? I would like that the upload field would be after comment textarea (between textarea & submit button). Is it possilbe?
Also any chance that you would remove the extra div with classes clear and clearfix. This is old school way of doing it. And I don’t even think these do anything as this plugin doesn’t have any CSS file so now you just have empty div there.
