Pick / choose an element

Hello!

I have to create a website for an events promoter.
The website will include a section where you, as a customer, can pick an artist you would like to perform at your event.
There’ll be a list with all the artists (+ descriptions) and I’d like it to have a little tickbox or arrow or something that will bring you to the contact form once you hit it, including the reference of that artist you chose.
So that when the promoter receives the e-mail, they will know which artist the customer picked.

Is there any plugin that I could use to achieve this?
