    did you try different options like Statically cached header and footer and Full page?
    I had similar issue and had to go with a Full page option.

    Ya, you must pay attention to the theme you use. Themes based on subsilver are table based and don’t respond to the CSS manipulation that PHBUnited does. Themes based on prosilver are CSS based and work just fine. I just switched out my subsilver choice for the prosilver and voila.

    Ah ok, i didnt know that. I will just have to find the right tables then and edit them myself since i rally wanna keep the theme i’m using right now.
