PHP Error via Query Monitor Admin Panel on WP Dashboard Broken

    I am getting a PHP Error via Query Monitor when using your plugin. Below is the error. Can you please investigate?

    Also, your dashboard plugin does not work on my WP Admin page. It seems to be broken.
