Photo upload stopped working

Not sure how long it’s been this way but just got word from user that photo upload has stopped working.
Everything seems to go according to design the image is accepted the progress bar shows progress and says the upload is successful, but no image is stored.
It isn’t in the public upload folder or anywhere else I can see.

Any ideas
