Phoney Users-Spam

I have Akismet enabled, I use a tabbed login/register widget, and I have captcha enabled, and I require all users to login with captcha every time they comne to my site, yet I continually have to go in and remove irrelevant articles by the dozens and delete phoney users every day.I dont want to have to keep spending 30-to 40 minutes a day o0n this. Can anyone help me out here?
