The login form on the page that you are linking to is not generated with our plugin. It’s ID is wpaloginform — maybe that helps you identify what plugin is creating it.
You can find this out (in Firefox for instance) by right-click and Inspect Element.
Regarding all email ending up in the junk folder, this has to do with your hosting email server.
You probably have spamming server neighbors and the whole server was marked a spam source.
Either ask them to move you on an email server that is not marked as a spam source.
Or bypass their email server altogether and use a different email server with a plugin such as:
