This looks like an incompatibility with the theme you are using (Diza).

I’ve contacted the theme authors to see if they are willing to collaborate to add compatibility between the theme and Fluid Checkout.

The compatibility issues with the theme Diza have been fixed and will be available with the next update 1.4.2 set to be released in the next week. You can also use the beta version 1.4.2-beta-17 from the following link:
