Payment Gateway Integration on a Page (Button)

Hi, I have a non-profit website (touchonelife.org) and I would like to add a ccavenue payment gateway on a page. Basically I want to create a page like this one https://www.teachforindia.org/donate/give-instantly?amount=500

I want a basic form where the user can enter his name, address, and donation amount and then when he clicks on submit, he is redirected to the ccavenue payment gateway site. I can make the form but I have no clue how to make that button work.

CCavenue has given a web integration kit for the same.
can we work this out?
