Payment form truncated on page

Hi, we are using your plugin on our KSPCA donations page, running WP 4.3.1 with the Enfold theme: . After filling the intitial form and get to the Pesapal form, it is truncated and only part of the first row shows on the page. I tried to place the shortcode [pesapal_donate] in the page with and without the layout editor, placing it within
tags, etc. I assume this may just be a simple CSS issue but was unable to identify the offending tag.Any thoughts please? Your assistance is greatly appreciated!
