Payment Form

I’m looking for a Payment form plugin where user fills out their information (customized by me, ex.. e-mail, date of arrival..etc. Also, I need to have at least 12 payment options (1 person – $12, 2 people – $22, 3 people – $30 etc) Basically I need it to be like a contact form plus the payment options. Any suggestions appreciated.
