Payment Error Message With BACS & Paypal Express

It would be great to have some insight into this problem of mine.
Issue 1: Paypal Express (cancel order, then re-place order)
The initial checkout process to the paypal express payment page is all fine – until a customer decides to click on “Cancel and Return to MatMade”.
It leads them to the review order page, and if they decide to go ahead with the order and click on “Place Order”, this error message appears.

However, for some reason, it stopped working and simply stopped loading the page with “error in processing order, please try again”.
However, the orders are still accounted for back-end as new orders. I’m befuddled and hope someone can help.
